Fendi Valdez n’avait que trois mois lorsque sa famille a remarqué que quelque chose n’allait pas : un large hématome était apparu sur son corps. À l’époque, très peu de professionnel·les de santé de République dominicaine connaissaient l’hémophilie, ce qui rendait son diagnostic difficile. Même en cas de diagnostic correct, le traitement était encore largement indisponible. Les concentrés de facteur étaient chers et difficiles à se procurer. Afin que son traitement dure plus longtemps, Fendi prenait parfois une dose inférieure à celle prescrite. À cause de ces soins limités, il avait fréquemment des saignements, manquait l’école, était hospitalisé, et a fini par développer des problèmes très graves aux deux genoux. Ce n’est que lorsque Fendi a commencé à recevoir des dons de facteur fournis par le Programme d’aide humanitaire de la Fédération mondiale de l’hémophilie qu’il a retrouvé l’espoir d’une vie meilleure.

Pour un enfant né avec une hémophilie sévère dans un pays sans approvisionnement fiable en traitement, la vie se mesure en saignements. Un choc, une chute, parfois un événement tout à fait mineur, et une articulation peut se remplir de sang. La douleur peut durer plusieurs jours, et au fil des années, les articulations se raidissent, ce qui conduit à des problèmes permanents de mobilité. Cela provoque alors des absences en cours ou au travail, et de longues périodes passées chez soi. Heureusement, ce cas de figure bien trop répandu chez les personnes atteintes d'hémophilie au Malawi s'améliore peu à peu grâce au soutien de la Fédération mondiale de l’hémophilie (FMH).

Pour Marlène Beijlevelt, infirmière spécialisée dans les soins des troubles de la coagulation au Centre médical universitaire d’Amsterdam (Pays-Bas), la prise de décision partagée va bien plus loin qu’une simple conversation entre patient·e et professionnel·le de santé. C’est un parcours continu qui rend les patient·es et les familles plus autonomes dans l’exploration de leurs choix et l’identification de leurs priorités, et qui leur permet de travailler en collaboration avec leur équipe de soins pour prendre les décisions les plus cohérentes avec leurs valeurs et besoins personnels.

Outils d’évaluation des capacités fonctionnelles

Les informations pertinentes recueillies dans le Registre mondial des troubles de la coagulation (RMTC) comprennent des données démographiques, des résultats cliniques et thérapeutiques, des paramètres concernant le fardeau de la maladie et les résultats déclarés par les patients eux-mêmes.

Dans son questionnaire élargi de collecte de données, le RMTC intègre 5 outils d’évaluation des capacités fonctionnelles :

Ensembles de données du RMTC sur l'hémophilie

Ensemble minimum de données du RMTC

L’Ensemble minimum de données du RMTC rassemble les informations de base (données démographiques, diagnostic, symptômes et traitements) collectées auprès des patients ayant accepté de participer au RMTC.

Ensemble élargi de données du RMTC

L’Ensemble élargi de données du RMTC correspond à un plus grand nombre d’informations collectées que celui de l’Ensemble minimum de données (données démographiques, diagnostic, symptômes, traitements et fardeau de la maladie). Disponible depuis mars 2019, l’Ensemble élargi de donnée du RMTC peut être utilisé par les Centres de traitement de l’hémophilie pour collecter des données plus détaillées sur les patients ayant accepté de participer au RMTC.
